SAGE journal International Journal of Care Coordination re-launching April 2014
Los Angeles, CA - Leading SAGE medical journal, the International Journal of Care Coordination, is being re-launched this month. Dedicated to expanding global understanding of care coordination, this peer reviewed journal will publish quarterly.
Care coordination focuses on creating patient-centered care that is more coordinated across care settings and over time, particularly for patients with long-term chronic and medically complex conditions who may find it difficult to ‘navigate’ often fragmented health care systems. There is no one model of care coordination but evidence suggests that the joined-up approach cross health and social care can achieve better results than those relying on single or limited set of strategies and this Journal will be the only dedicated Journal in this field of research.
The International Journal of Care Coordination(formerly published as the International Journal of Care Pathways) provides an international forum for the latest scientific research in care coordination. The journal accepts papers from a range of national and international health organizations. The Journal offers insightful overviews and reflections on innovation, underlying issues, and thought-provoking opinion pieces in related fields. Articles from multidisciplinary fields are welcomed from leading health care academics and policy-makers. Published articles types include original research, reviews, guidelines papers, book reviews, and news items.
